An extension of Shawnee Mission Medical Center’s (SMMC) Behavioral Health Department, the home health program is specially designed for patients who need more than outpatient care but do not require hospitalization. Care plans are directed by each patient’s own physician. SMHHC behavioral health nurses Patty Clayborn, RN, and Roxana Coslett, RN, BC, then work with patients in their everyday environments to effectively execute these plans and educate families on how to support their loved ones through the process.
“We are the bridge from the patient to the doctor,” said Clayborn. “We want to keep patients out of the hospital and help give them power and control of their own lives.”
