Congestive Heart Failure Program

Heart failure patients and their families benefit from partnering with our health care team to aggressively manage their disease. Primary care physicians, cardiologists, clinical nurse specialists, nurses, social workers, dieticians, therapists, pharmacists, chaplains and home health care professionals work together to develop a treatment plan that addresses:

When appropriate, heart failure patients benefit from home care cardiac specialty nurses and/or home remote telephone monitoring systems. Empowering patients and their families with adequate information and resources improves quality of life and minimizes the need for hospitalization.
