When you have requested an appointment, one of our team members will contact you to schedule a consultation. In order for our physicians to review your file and be prepared for your consultation, we require that the following forms be filled out and faxed or mailed to us prior to your appointment.
So that you have an idea of insurance coverage, our billing experts will contact your insurance company to explore your reproductive medicine benefits. We will review these benefits with you at your first visit.
It is also important that we receive your previous medical records to ensure we have detailed information for our clinical team. We will notify you if we have not received your records.
At your first appointment, the physician will review your records and history and provide you with the next steps for evaluation and treatment planning.
Notice to Parents
Please note that if you plan to bring a child to your appointment, you will need to make arrangements for a friend or relative to accompany your child. We are asking that they remain in the waiting room during your visit. Per office policy, children are not allowed in clinic areas. Not only do we care about your children and have a moral and legal obligation for their safety, but this also allows our staff to give you their undivided attention, which will maximize your visit.
